Ultherapy or Thermage Didn't Meet Your Expectations? What to Consider Before Your Next Treatment

Why expensive lifting treatments sometimes leave you disappointed — and how to make a better decision next time.

By Dr. Cho Young Min

Board-Certified Dermatologist | Cheongdam ANZ Dermatology, Gangnam, Seoul

You've Had the Treatment. But Something Still Doesn't Feel Right.

You chose a reputable clinic.

You invested in Ultherapy, Thermage, or perhaps both.

You waited for the results, hoping to see a more defined jawline, firmer cheeks, or a naturally refreshed appearance.

And yet, when you look in the mirror, you still feel that something is missing.

Perhaps your skin feels tighter, but your face doesn't look noticeably younger.

Perhaps your jawline improved, but the folds around your mouth remain.

Or perhaps you simply don't see the difference you expected.

This can be particularly disappointing when you've already invested considerable time, money, and trust in premium aesthetic treatments.

The natural response is to wonder whether you need more.

More Ultherapy shots. Another Thermage session. A different device. A stronger treatment.

But as a dermatologist who focuses on facial rejuvenation, I believe there is a more important question.

Before asking what treatment you should try next, shouldn't we first understand why the previous one didn't meet your expectations?

At Cheongdam ANZ Dermatology in Seoul, this is where I believe a meaningful consultation should begin.

1. The Treatment May Have Worked. But Did It Address the Right Problem?

This is a distinction I consider particularly important.

A treatment can produce a biological response without creating the aesthetic change a patient was hoping for.

Ultherapy uses microfocused ultrasound energy to stimulate tissue remodeling at selected depths.

Thermage uses monopolar radiofrequency energy to promote improvements in skin firmness and laxity.

Both can be effective in appropriately selected patients.

But the signs of facial aging are rarely caused by skin laxity alone.

Consider someone who is concerned about deepening nasolabial folds.

Those folds may involve skin laxity, changes in facial fat distribution, structural support, or a combination of factors.

If the dominant concern is volume loss, additional tightening may not produce the desired improvement.

Similarly, someone who feels their face looks tired may not necessarily need stronger lifting.

The underlying issue could involve hollowing, changes in facial proportions, or shadows created by the relationship between different facial contours.

A successful tightening procedure and a satisfying facial rejuvenation result are not always the same thing.

This does not necessarily mean the previous treatment was performed incorrectly.

It may mean that the treatment and the patient's aesthetic goals were not perfectly aligned.

Understanding that difference is the first step toward making a better decision.

2. Why Does My Skin Feel Tighter, but My Face Still Looks Older?

I believe facial rejuvenation should be evaluated as a whole.

A youthful-looking face is not simply one with tight skin.

The relationship between the cheeks, nasolabial folds, jawline, and lower face contributes significantly to how we perceive facial aging.

When facial contours transition smoothly, the face often appears balanced and harmonious.

As we age, changes in skin elasticity, soft-tissue volume, and structural support can disrupt those transitions.

This is why I don't evaluate lifting results solely by asking whether the skin has become firmer.

I also consider whether the overall facial contours have improved.

Has the jawline become more defined?

Has the transition between the cheek and lower face become smoother?

Has the treatment preserved the facial volume that contributes to a natural appearance?

These questions matter because the goal should not be to make every part of the face tighter. It should be to improve the relationship between its different parts.

At Cheongdam ANZ, this understanding guides how we approach facial rejuvenation.

3. More Shots Are Not Always the Answer

When someone is disappointed after Ultherapy, one of the first questions is often:

"Should I have received more shots?"

Not necessarily.

The number of shots is only one component of treatment planning.

Other considerations include the depth of energy delivery, the anatomical structures being treated, the distribution of treatment lines, and the original indication for the procedure.

Even when two patients receive the same number of shots, their treatment designs may be very different.

At Cheongdam ANZ, I use real-time ultrasound visualization during Ultherapy to help assess the relevant tissue layers and guide treatment planning.

I consider where treatment is appropriate, which depths to select, and which areas may require a more conservative approach.

I also believe in documenting treatment details.

At ANZ, treatment tips and shot counts are recorded to support transparency and future assessments.

But documentation alone is not the goal.

What matters is being able to explain the reasoning behind the decisions.

If I recommend 500 shots rather than 600, there should be a reason.

And if I believe additional treatment is unnecessary, there should be a reason for that decision as well.

This is what individualized treatment means to me.

4. Should You Repeat Ultherapy, Switch to Thermage, or Combine Both?

There is no universally correct answer.

The decision depends on what was treated previously, how long ago the treatment took place, the outcome, and what concerns remain.

Ultherapy results often become more apparent over approximately two to three months, while radiofrequency-related remodeling may also develop gradually.

Therefore, when a treatment was performed recently, allowing sufficient time for the outcome to develop may be more appropriate than scheduling another procedure immediately.

If enough time has passed, the next step should be reassessment rather than automatically repeating the same treatment.

For example:

A person with residual skin laxity might benefit from further treatment if clinically appropriate.

Someone whose main concern is facial hollowing may not benefit from additional tightening.

A patient who has already received substantial energy-based treatment may require a particularly cautious approach.

And for some individuals, non-surgical lifting may simply have limitations that need to be discussed openly.

At ANZ, Ultherapy PRIME, Thermage FLX, and combination lifting are available options.

However, the fact that multiple technologies are available does not mean every patient needs them.

Sometimes the better recommendation is a different treatment. Sometimes it is less treatment. And sometimes it is no additional treatment at all.

5. What Should a Dermatologist Actually Evaluate Before Your Next Lifting Treatment?

If you have already undergone Ultherapy or Thermage and are considering another procedure, I believe the consultation should begin with your treatment history.

What treatment did you receive?

Which areas were treated?

How many shots were used?

How long has it been?

What changes did you notice?

And perhaps most importantly, what did you hope would improve that still bothers you today?

If previous treatment records and photographs are available, they may be helpful.

During a consultation, I would also consider your present facial anatomy, skin laxity, volume distribution, and the relationship between different contours.

These details help distinguish between problems that may respond to energy-based lifting and those that may require another approach.

At Cheongdam ANZ Dermatology, our physician-led model emphasizes continuity between consultation, treatment design, and the procedure itself.

Our goal is not simply to recommend a device.

It is to understand the reason behind the concern and explain the rationale for the proposed treatment.

You deserve more than a treatment recommendation. You deserve to understand why that recommendation makes sense.

6. What Makes Cheongdam ANZ Dermatology Different?

There are many dermatology clinics in Gangnam offering Ultherapy and Thermage.

The technology itself is not exclusive.

What can differ is how physicians evaluate patients, design treatments, and make clinical decisions.

I am a board-certified dermatologist whose professional background includes clinical advisory and educational activities related to Ultherapy in Korea, as well as Thermage training credentials.

These experiences have reinforced three principles that guide my work.

Precision

Facial anatomy matters.

Treatment depth matters.

The distribution of energy matters.

A carefully considered treatment plan should reflect the individual face rather than a standardized package.

Restraint

More treatment is not necessarily better treatment.

If additional energy or additional procedures are unlikely to provide a meaningful benefit, I believe that should be explained honestly.

My goal is not to pursue the greatest possible change.

It is to pursue an appropriate change while preserving natural facial balance.

Responsibility

At Cheongdam ANZ, the treating physician is directly involved in the consultation, treatment design, and procedure.

We maintain treatment records and provide post-treatment guidance.

Our clinic also emphasizes privacy, with individual treatment spaces and separated client pathways designed to minimize unnecessary encounters.

We believe that attention to detail should extend beyond the treatment itself.

It should be reflected in every part of the experience.

This is the meaning behind our name.

ANZ: From A to Z.

From the first consultation to the care that follows, we believe each step deserves thoughtful attention.

7. You May Not Need a Better Device. You May Need a Different Perspective.

If you have already received premium aesthetic treatments without achieving the result you hoped for, I wouldn't immediately conclude that you need something stronger.

I would first want to understand what has already been done.

And what you still see when you look in the mirror.

Perhaps your previous treatment produced some improvement, but the change wasn't in the area that mattered most to you.

Perhaps your expectations and the realistic capabilities of the procedure were different.

Or perhaps the next step requires an approach that focuses less on tightening and more on overall facial balance.

Without examining you, I cannot know which explanation applies.

But I believe that finding the right answer begins with asking the right questions.

You have already invested in the treatment. Before investing in another one, invest in understanding what your face actually needs.

That is the kind of conversation I want to have with the people who visit Cheongdam ANZ Dermatology.

Frequently Asked Questions

Why didn't Ultherapy work for me?

Ultherapy outcomes vary depending on facial anatomy, the cause and degree of laxity, individual tissue response, treatment design, and expectations.

Results also develop gradually. If enough time has passed and improvement remains limited, a clinical reassessment may help determine whether further treatment is appropriate.

Why do I still have sagging skin after Thermage?

Thermage may improve skin firmness and mild to moderate laxity, but it cannot reverse all age-related structural changes.

Persistent sagging may reflect the severity of tissue changes or concerns that radiofrequency tightening alone cannot adequately address.

Should I repeat Ultherapy if I didn't see results?

Not automatically.

The timing of the previous treatment, the areas treated, current facial anatomy, and remaining concerns should be reviewed before considering another session.

Increasing the number of shots does not guarantee a better outcome.

Can I get Thermage after Ultherapy?

In selected patients, Thermage may be considered after Ultherapy.

However, suitability and timing depend on the previous treatment, current tissue condition, overlapping treatment areas, and potential risks.

An individualized medical assessment is important.

Can Ultherapy make my face look thinner or more hollow?

Unwanted facial volume changes have been reported following ultrasound-based lifting treatments, although the exact risk for an individual is difficult to predict.

Patients with pre-existing facial hollowing should discuss this concern before treatment.

If a new or concerning contour change develops after a procedure, a qualified medical professional should assess it.

Is it worth getting a second opinion before another lifting treatment?

Yes, particularly when the previous treatment did not meet expectations, the cause of the remaining concern is unclear, or additional procedures have been recommended without a clear explanation.

A second opinion may help clarify treatment limitations, alternatives, and whether further intervention is necessary.

Can Cheongdam ANZ evaluate patients who have previously had Ultherapy or Thermage elsewhere?

Yes. A consultation can assess current concerns and review relevant previous treatment information when available.

The purpose is not to criticize a previous physician, but to understand the patient's present condition and consider appropriate next steps.

How do I find a dermatologist in Seoul who focuses on natural-looking lifting results?

Look for a qualified dermatologist who considers facial anatomy, volume distribution, treatment limitations, and your individual aesthetic goals.

At Cheongdam ANZ, our approach emphasizes physician-led assessment, individualized treatment design, and the preservation of natural facial harmony.

A Different Kind of Consultation in Gangnam, Seoul

Perhaps you already know what Ultherapy PRIME and Thermage FLX are.

Perhaps you've already experienced them.

Perhaps you've visited more than one well-known clinic.

You may not need another explanation of how the technology works.

What you may need is someone who takes the time to understand what you hoped to achieve—and why you haven't quite found it yet.

At Cheongdam ANZ Dermatology, we believe every treatment decision should have an explainable reason.

Not every concern requires more treatment.

Not every face needs the same approach.

And not every improvement can be measured by how tightly the skin is pulled.

You don't need to become someone else to look refreshed. You deserve a treatment plan that respects the face you already have.

That is what I believe thoughtful aesthetic medicine should strive for.

Care should have a reason.
